Survey Sweepstakes Official Rules

NO PURCHASE NECESSARY TO ENTER OR WIN. A PURCHASE WILL NOT INCREASE YOUR CHANCES OF WINNING.

1. Sponsor
Parcel Health, Inc., Pittsburgh, Pennsylvania ("Sponsor").

2. Eligibility
Open to legal residents of the fifty (50) United States and the District of Columbia who are 18 years of age or older at the time of entry. Employees, officers, and directors of Sponsor and their immediate family members (spouse, parent, sibling, child) and household members are not eligible. Void where prohibited by law.

3. Sweepstakes Period
The Sweepstakes begins on the date the survey is published and ends at 11:59 PM Eastern Time on December 31, 2026 ("Sweepstakes Period"). Sponsor's computer is the official timekeeping device.

4. How to Enter
During the Sweepstakes Period, visit parcelhealth.co and complete the Tully Tube Patient Experience Survey in full. At the end of the survey, opt in to the sweepstakes by providing a valid email address. Limit one (1) entry per person. Incomplete survey submissions will not be eligible.

5. Drawing Mechanic and Winner Selection
Winners are determined by response threshold as follows:

Total Completed Responses

• 10 - 24 responses = 1 winner
• 25 - 49 responses = 3 winners
• 50 - 99 responses = 5 winners
• 100 or more responses = 10 winners

When a new threshold tier is reached, Sponsor will conduct a random drawing from all eligible entries within thirty (30) days. Each tier's drawing is cumulative — all eligible entries received to date are included in each drawing. Previously selected winners remain winners and are not re-entered. If the Sweepstakes Period ends on December 31, 2026 before a threshold is reached or before an additional threshold is crossed, Sponsor will conduct a final drawing within thirty (30) days of that date using whatever entries have been collected at that time, awarding the number of winners corresponding to the highest threshold tier reached.

6. Prize
Each winner receives one (1) $60 gift card (type at Sponsor's discretion). Approximate retail value: $60.00. Maximum total prize pool: $600.00. Prizes are non-transferable and no substitution or cash equivalent is permitted except at Sponsor's sole discretion. Sponsor reserves the right to substitute a prize of equal or greater value.

7. Odds of Winning
Odds depend on the total number of eligible entries received and the threshold tier reached at the time of each drawing.

8. Winner Notification
Winners will be notified by email within thirty (30) days of the applicable drawing. Winners must respond within forty-eight (48) hours of notification or the prize will be forfeited and an alternate winner may be selected. Sponsor is not responsible for undelivered, misdirected, or delayed email notifications.

9. Privacy
Survey responses are collected anonymously. Email addresses collected for sweepstakes entry are used solely for winner notification and administration of this Sweepstakes. Sponsor will not sell or share email addresses with third parties. See Sponsor's Privacy Policy at parcelhealth.co for full details.

10. General Conditions
Sponsor reserves the right to cancel, suspend, or modify the Sweepstakes if fraud, technical failures, or any other factor impairs the integrity of the Sweepstakes, as determined by Sponsor in its sole discretion. Sponsor reserves the right to extend the Sweepstakes Period at its discretion with appropriate notice. By entering, participants agree to be bound by these Official Rules and the decisions of the Sponsor, which are final and binding in all matters relating to this Sweepstakes.

11. Governing Law
This Sweepstakes is governed by the laws of the Commonwealth of Pennsylvania, without regard to conflict of law principles.
